 Types of Support

Residential placement (group home / supervised living)

AFL (family-style placement)

AFL (Alternative Family Living) provides care in a private home with a trained caregiver. Individuals live in a family-style environment that promotes comfort and stability. Care is highly personalized to match each individual’s needs and preferences. Caregivers assist with daily living activities, supervision, and emotional support. The setting encourages strong relationships and a sense of belonging. It offers a more natural and less institutional living experience. This model supports independence while maintaining a high level of care and safety.

Community Integration (COI)

Community Integration focuses on helping individuals become active participants in their community. Services include support with life skills, social interaction, and daily activities. Individuals are encouraged to engage in recreational, educational, and social opportunities. Staff provide guidance and supervision to ensure safety and success. The goal is to build confidence, independence, and meaningful connections. Programs are tailored to each individual’s interests and abilities. This service enhances quality of life through community involvement and personal development.

Supported living

Supported living is designed for individuals who are able to live in a more independent setting. Services are flexible and customized based on each person’s unique needs. Support may include assistance with daily activities, routines, and life skills. Individuals maintain control over their living environment and personal decisions. Staff provide guidance, supervision, and encouragement when needed. This model promotes independence while ensuring safety and consistency of care. It is ideal for those seeking a balance between freedom and structured support.

Family-based caregiver option (if eligible)

This option allows individuals to receive care from a qualified family member in their own home. It supports continuity of care in a familiar and comfortable environment. Caregivers are required to complete training and follow established care guidelines. Services include assistance with daily activities, supervision, and documentation. The program ensures accountability through structured monitoring and compliance. Eligibility and participation are determined based on program requirements. This option strengthens family involvement while maintaining professional care standards.

Medication Management

Medications are managed by trained staff in accordance with established policies and procedures to ensure proper administration, documentation, and monitoring.
